Bay Animal Clinic now offers patient forms online so you can complete them in the convenience of your own home or office.

New Patient Registration Form - Required

This lets us record our pet and owners into our practice management software so that we can effectively and efficiently provide the medical and customer service that you and your loved ones deserve.

Boarding Release and Medical Information Form - Required

The Boarding Release Form is required for all clients wishing to board their loved ones at Bay Animal Clinic. The boarding release form only needs to be filled out once. Once we have it on file, you can board as many times as you'd like. The Medical Information Form only needs to be filled out if Bay Animal Clinic will be administering treatment or medications while your baby is boarding with us. This helps us monitor changes in medications and treatment plans provided by other veterinary professionals. We truly appreciate your compliance. These forms allow us to provide the best service possible for you and your pets.
